Step 1: Contact Our Office

Our preferred method of communication is email. Jacqui has a very busy clinical schedule during business hours Tuesdays-Fridays,

she is often able to respond quicker to an email than a phone call in between clients. Monday is an administrative day and generally the best day to reach her for inquiries.

Things to include in your email:​

  • the nature of your inquiry and reason for seeking treatment

  • if you were referred by someone

  • your preferred days and times Tuesday-Friday that you are available for appointments

  • the best method and time to contact you, by text email or phone.

If you have questions or would like to discuss your circumstance by phone prior to booking, we can set up a time to briefly consult. Please send an email or text to set up a time to schedule a phone call. We recommend that you set up an a number of treatments,  most often scheduled 1 week apart, when initially booking. In our experience, it is most beneficial to receive a series of treatments without too much time in between- which may not be possible if not planned ahead.

Step 2: Completing Forms

After booking  your appointment, you will receive a welcome letter, and the option to set up a password to her patient portal through the practice management software Jane. 

​

you will receive a detailed health history form 

policies and consents that you can fill out online and submit prior to your appointment. 

​

Please also include any other medical reports or information that may be useful in your care. 

​

Jacqui will review this information before your first visit, where she will do a in depth health history review and consultation prior as well as initial treatment.
